ABSTRACT

Introdução: As lesões de nervos periféricos são mais frequentes em membros superiores (MMSS), que são importantes para as atividades de vida diárias, equilíbrio e reflexos de proteção. Alterações no sistema de controle postural podem impactar na funcionalidade desses indivíduos. Portanto, é necessário alternativas para melhorar as alterações secundárias à lesão. Objetivo: Avaliar os efeitos de um protocolo de exercícios de força, flexibilidade e equilíbrio em paciente com lesão de nervos periféricos. Métodos: Estudo de caso de abordagem quantitativa realizado na Clínica Escola de Fisioterapia da Unochapecó, entre abril e julho de 2021, totalizando 30 intervenções de 60 minutos cada. A amostra foi constituída por um paciente do sexo masculino, 46 anos com diagnóstico de lesão de nervos periféricos no MMSS direito, decorrente de um acidente motociclístico. Foi realizada avaliação inicial, aplicação do protocolo, após a realização das escalas e testes: BESTest, MiniBESTest, Romberg-Barré, Dinamometria manual isométrica, Teste de Sentar e Alcançar e Mini-Exame do Estado Mental. O protocolo continha exercícios de força, flexibilidade e equilíbrio. Os dados foram analisados através de estatística descritiva comparativa, antes e após o protocolo (média e porcentagem). Resultados: Houve melhora na força muscular, flexibilidade toracolombar (aumento de 13 cm "61,90%") e testes de equilíbrio (15,81% BESTest, 21,42% MiniBESTest e 67,16% Romberg-Barré). Conclusão: O protocolo de exercícios refletiu positivamente na melhora da flexibilidade, força muscular e equilíbrio do paciente, tornando-se uma alternativa viável para melhorar as alterações secundárias a lesão. ABSTRACT

SUMMARY: This study aimed to accurately localize the location and depth of the centre of the highest region of muscle spindle abundance (CHRMSA) of the triceps brachii muscle. Twenty-four adult cadavers were placed in the prone position. The curve connecting the acromion and lateral epicondyle of the humerus close to the skin was designed as the longitudinal reference line (L), and the curve connecting the lateral and the medial epicondyle of the humerus was designed as the horizontal reference line (H). Sihler's staining was used to visualize the dense intramuscular nerve region of the triceps brachii muscle. The abundance of muscle spindle was calculated after hematoxylin and eosin stain. CHRMSA was labelled by barium sulphate, and spiral computed tomography scanning and three- dimensional reconstruction were performed. Using the Syngo system, the projection points of CHRMSA on the posterior and anterior arm surface (P and P' points), the position of P points projected to the L and H lines (PL and PH points), and the depth of CHRMSA were determined. The PL of the CHRMSA of the long, medial, and lateral heads of the triceps brachii muscle were located at 34.83 %, 75.63 %, and 63.93 % of the L line, respectively, and the PH was located at 63.46 %, 69.62 %, and 56.07 % of the H line, respectively. In addition, the depth was located at 34.73 %, 35.48 %, and 35.85 % of the PP' line, respectively. These percentage values are all the means. These body surface locations and depths are suggested to be the optimal blocking targets for botulinum toxin A in the treatment of triceps brachii muscle spasticity.


RESUMEN: Este estudio tuvo como objetivo localizar con precisión la ubicación y la profundidad del centro de la región más alta del huso muscular (CHRMSA) del músculo tríceps braquial. Se colocaron veinticuatro cadáveres adultos en posición prona y se designó la curva que conecta el acromion y el epicóndilo lateral del húmero cerca de la piel como la línea de referencia longitudinal (L), y la curva que conecta los epicóndilos lateral y medial del húmero fue designada como la línea de referencia horizontal (H). Se usó la tinción de Sihler para visualizar la región nerviosa intramuscular densa del músculo tríceps braquial. La abundancia de huso muscular se calculó después de la tinción con hematoxilina y eosina. CHRMSA se marcó con sulfato de bario y se realizó una tomografía computarizada espiral y una reconstrucción tridimensional. Usando el sistema Syngo, fueron determinados los puntos de proyección de CHRMSA en la superficie posterior y anterior del brazo (puntos P y P'), la posición de los puntos P pro- yectados en las líneas L y H (puntos PL y PH) y la profundidad de CHRMSA. Los PL de la CHRMSA de las cabezas larga, medial y lateral del músculo tríceps braquial se ubicaron en el 34,83 %, 75,63 % y 63,93 % de la línea L, respectivamente, y el PH se ubicó en el 63,46 %, 69,62 %, y 56,07 % de la línea H, respectivamente. La profundidad se ubicó en el 34,73 %, 35,48 % y 35,85 % de la línea PP', respectivamente. Estos valores porcentuales son todas las medias. Se sugiere que estas ubicaciones y profundidades de la superficie corporal son los objetivos de bloqueo óptimos para la toxina botulínica A en el tratamiento de la espasticidad del músculo tríceps braquial.

RESUMEN: El músculo tríceps braquial, es el motor primario para el movimiento de extensión de codo, por lo que una lesión que afecte su función perjudicaría enormemente la calidad de vida de los afectados. El conocimiento de su inervación y la localización biométrica de sus puntos motores, es una herramienta útil en terapias de electro estimulación muscular. El objetivo del estudio fue determinar el número y localización de los puntos motores de este músculo. Para ello, se utilizaron 30 miembros superiores de individuos brasileños, a los cuales se les realizó una disección detallada del compartimiento posterior del brazo. Se registró el número de ramos, puntos motores y localización biométrica de cada uno de los ramos destinados a las cabezas del músculo triceps braquial. Se utilizó como punto de referencia una Línea biepicondilar, trazada entre los epicóndilos humerales. En todos los casos este músculo estaba inervado por el nervio radial. El promedio de puntos motores (PM) para la cabeza larga del músculo (CL) fue de 3,9 ± 1,4; 4,8 ± 1,2 para la cabeza medial (CM) y 4,1 ± 1,4 para la cabeza lateral (CLat). Los puntos motores se concentraron preferentemente en el tercio medio del brazo, tanto a nivel general, como también por cada cabeza. Los datos biométricos aportados complementarán el conocimiento de la inervación de este músculo y favorecerá una mejor comprensión y elección de tratamientos frente a una patología.


SUMMARY: The triceps brachii muscle is the primary motor for elbow extension movement, so a lesion that affects its function would greatly harm the quality of life of those affected. The knowledge of its innervation and the biometric localization of its motor points is a useful tool in electro-stimulation muscular therapies. The objective of the study was to determine the number of branches and location of the motor points of this muscle. To this end, 30 superior members of Brazilian individuals were used, to whom a detailed dissection of the posterior compartment of the arm was performed. The number of branches, motor points and biometric location of each of the branches destined for the three heads of the brachial triceps muscle was recorded. A biepicondilar line, traced between the humeral epicondyles, was used as a reference point. In all cases, this muscle was innervated by the radial nerve. The average motor points for the long head of the muscle (LH) was 3.9 + 1.4; for the medial head (MH) was 4.8 + 1.2 and for the lateral head (LatH) was 4.1+1.4. The motor points were concentrated mainly in the middle third of the arm, both at a general level, and also for each head. The biometric data provided will complement the knowledge of the innervation of this muscle and will favor a better understanding and choice of treatments for a pathology.

ABSTRACT

The radial nerve is a terminal branch of the brachial plexus that innervates the entire posterior compartment of the upper limb. Researches have shown that the branches can contribute to innervation of the muscles from the anterior compartment of the arm. This study sought to identify branches of radial nerve for brachialis and coracobrachialis muscles and its clinical value during surgery. Dissection was performed in 60 arms, from 30 corpses selected from the Human Anatomy Laboratories of Universidade Estadual de Ciências da Saúde de Alagoas (UNCISAL). In each arm, three segments of the same proportion were considered (proximal, middle and distal) in order to observe the number of radial nerve branches to brachialis and coracobrachialis muscles. Branches were identified in 4 of the 60 upper limb (6.66%) with the following distribution: nerve with 1 branch to the brachialis muscle in the distal third (1.66%); with 1 branch to the middle third of the coracobrachialis (1.66%) and 5 branches to the coracobrachialis muscle in the proximal third; 1 branch being found in an upper limb (1.66 %) and 4 branches in the other upper limb (1.66 %). The event was not bilaterally in the same body, and in four cases there was no branching, but only communication with other terminal nerves of the brachial plexus. In conclusion, although most authors do not describe the possibility of the occurrence of radial nerve branches to the muscles of the anterior compartment of the arm, this condition exists as an anatomical variation.


El nervio radial es un ramo terminal del plexo braquial que inerva el compartimiento posterior del miembro superior. Diversos estudios han mostrado que sus ramos pueden contribuir a la inervación de los músculos del compartimiento anterior del brazo. El presente estudio tuvo como objetivo identificar ramos del nervio radial dirigidos a los músculos braquial y coracobraquial y su importancia clínica durante las cirugías. Para ello, se disecaron 60 brazos de 30 cuerpos formolizados en los Laboratorios de Anatomía Humana de la Universidad Estadual de Ciencias de la Salud de Alagoas, Brasil (UNCISAL). En cada brazo, se consideraron tres partes de similares proporciones (proximal, medio y distal) para localizar los ramos dirigidos a los músculos mencionados. En 4 de los 60 brazos (6,66%) se identificaron ramos con la siguiente distribución: un ramo para el músculo braquial en el tercio distal (1,66%); un ramo para el tercio medio del músculo coracobraquial (1,66%) y cinco ramos para este mismo músculo en su tercio proximal; en otro miembro (1,66%)se encontró solo un ramo y en la otra muestra (1,66%) se encontraron cuatro ramos. Ninguno de los casos fue bilateral y en los cuatro casos no había ninguna ramificación. No hubo comunicación con el nervio musculocutáneo pero si con otros ramos del plexo braquial. Aunque la mayoría de los autores no describe la posibilidad de que el nervio radial inerve a los músculos anteriores del brazo, esta disposición existe como una variación anatómica.

ABSTRACT

Variations of the terminal branches of the brachial plexus in the upper limb are abundant and frequently have been reported. But connection between its radial and ulnar nerves at humeral level is very rare. During routine dissection of the right arm of a 75-year-old male cadaver in the Department of Anatomy of Birjand University of Medical Sciences (BUMS), in Iran, we observed a connection between radial and ulnar nerves, and from the literature reviewed, we found only two cases that had been reported previously, one of them was unilateral and the other bilateral. Awareness of such variations are very important for surgeons and diminish the possible complication of regional anesthesia and surgical procedures, hence, we decided to report it.


Las variaciones de los ramos terminales del plexo braquial en el miembro superior son abundantes y reportadas con frecuencia. Sin embargo, la conexión entre los nervios radial y ulnar a nivel humeral es muy rara. Durante la disección rutinaria de un brazo derecho en un cadáver de sexo masculino de 75 años, en el Departamento de Anatomía de la Birjand University of Medical Sciences (BUMS) en Irán, se observó una relación entre los nervios radial y ulnar. En la literatura, encontramos sólo dos casos que habían sido reportados previamente, uno bilateral y otro unilateral. Conocer estas variaciones es de importancia para los cirujanos, así como para disminuir posibles complicaciones en la colocación de anestesia regional y procedimientos quirúrgicos.

ABSTRACT

A general analysis of the behaviour of “Cebus” shows that when this primate moves position to feed or perform another activity, it presents different ways of locomotion. This information shows that the brachial biceps muscle of this animal is frequently used in their locomotion activities, but it should also be remembered that this muscle is also used for other development activities like hiding, searching for objects, searching out in the woods, and digging in the soil. Considering the above, it was decided to research the histoenzimologic characteristics of the brachial biceps muscle to observe whether it is better adpted to postural or phasic function. To that end, samples were taken from the superficial and deep regions, the inserts proximal (medial and lateral) and distal brachial biceps six capuchin monkeys male and adult, which were subjected to the reactions of m-ATPase, NADH-Tr. Based on the results of these reactions fibres were classified as in Fast Twitch Glycolitic (FG), Fast Twitch Oxidative Glycolitic (FOG) and Slow Twitc (SO). In general, the results, considering the muscle as a whole, show a trend of frequency FOG> FG> SO. The data on the frequency were studied on three superficial regions FOG=FG>SO; the deep regions of the inserts proximal FOG=FG=SO and inserting the distal FOG>FG=SO. In conclusion, the biceps brachii of the capuchin monkey is well adapted for both postural and phasic activities.


Uma análise geral do comportamento do “Cebus apella” mostra que este primata quando desloca para se alimentar ou realizar outra atividade apresenta diferentes maneiras de locomoção. Estas informações mostram que o músculo bíceps braquial deste animal é usado freqüentemente nas suas atividades de locomoção, mas deve ser lembrado ainda que este músculo é usado também para desenvolvimento de outras atividades como esconder-se, procurar objetos, vasculhar arboredos, além de cavar o chão. Considerando-se o exposto acima decidiu-se pesquisar as características histoenzimológicas do músculo bíceps braquial do macaco-prego com o objetivo de comparar se este músculo esta melhor adaptado para funções posturais ou fásicas. As amostras foram retiradas das regiões superficiais e profundas; inserções proximais (medial e lateral) e distal de seis macacos-prego machos e adultos, os quais foram submetidos às reações de m-ATPase, NADH-Tr. Baseado nos resultados das reações, as fibras foram classificadas em Fast Twitch Glycolitic (FG), Fast Twitch Oxidative Glycolitic (FOG) e Slow Twitch (SO). Quanto à área dos diferentes tipos de fibras, os resultados encontrados foram semelhantes em todas as amostras estudadas, e as fibras de contração rápida foram sempre maiores do que as de contração lenta (FG=FOG>SO). Os dados obtidos sobre a frequência foram: nas três regiões superficiais estudadas FOG=FG>SO; nas regiões profundas das inserções proximais FOG=FG=SO e na inserção distal FOG>FG=SO. Baseado nestes dados pode-se concluir que o músculo bíceps braquial do macaco-prego está bem adaptado tanto para atividades posturais como fásicas.

ABSTRACT

A thorough knowledge of the variations in the patterns of muscle insertion and the consequent neurovascular entrapment due to them is important for orthopedic and plastic surgeons. During routine dissection, we encountered a unique occurrence of an accessory brachialis muscle (acBr) blending with the medial aspect of bicipital aponeurosis in the cubital fossa. It also entrapped the median nerve and brachial artery due to its superficial position. The abnormality reported here might result in neurovascular compression symptoms in the upper limb and possible mechanical advantages or disadvantage in the flexion of elbow joint. To the best of our knowledge, the brachialis variant insertion we present here is rare in the list of its anomalies. We also discuss in this report the embryological background and the clinical application of the variation that can abet clinicians in evaluating symptoms involving the upper limb.


Es importante para los cirujanos ortopédicos y plásticos, el conocimiento integral de las variaciones en los patrones de inserción de los músculos y el consiguiente atrapamiento neurovascular. Durante una disección de rutina, encontramos un caso único de un músculo accesorio braquial con aponeurosis bicipital en el lado medial de la fosa cubital. También cubre la arteria braquial y nervio mediano debido a su posición superficial. La anomalía informada aquí podría resultar en síntomas de compresión neurovascular en el miembro superior y las posibles ventajas o desventajas mecánicas de flexión de la articulación del codo. De acuerdo a lo estudiado, la inserción variante braquial que presentamos aquí es un caso raro dentro de la lista de sus variaciones. También se discuten en este informe la base embriológica y la aplicación clínica de la variación que puede ser útil para los clínicos en la evaluación de los síntomas relacionados con el miembro superior.

ABSTRACT

OBJECTIVES: Variations such as communications between the median nerve and musculocutaneous nerve or in their abnormal branching pattern constitute a major concern in clinical and surgical field. Knowledge of these variations not only provides the clinician with a proper interpretation of the case, but also minimizes the complication in surgical approaches in this region. METHOD: We examined 50 isolated upper limbs to investigate the possible incidences of various types of communications between these two neighbouring peripheral nerves. RESULT: Twenty-eight per cent of limbs were found to have communication between these two nerves. When categorized according to Venieratos and Anagnostopoulou's classification method, 11 out of 14 cases (79%) showed type I communications, two out of 14 (14%) showed type II and the remaining one (7%) showed type III communication pattern. CONCLUSION: Prior knowledge of communications between these two neighbouring nerves, both in terms oftheir incidences and pattern ofcommunications, may be ofconsiderable significance to neurologists and orthopaedic surgeons in dealing with nerve entrapment syndromes in the upper limb of patients.


OBEJTIVOS: Las variaciones tales como las comunicaciones entre el nervio mediano y el nervio musculocutáneo o en su patrón de ramificación anormal constituyen un interés principal en el campo clínico y quirúrgico. El conocimiento de estas variaciones no sólo proporciona una interpretación adecuada del caso clínico, sino que también minimiza la complicación en abordajes quirúrgicos en esta región. MÉTODO: Se analizaron 50 miembros superiores aislados para investigar las posibles incidencias de diversos tipos de comunicaciones entre estos dos nervios periféricos vecinos. RESULTADO: Se halló que el veintiocho por ciento de los miembros tenía comunicación entre estos dos nervios. Cuando fueron categorizados según el método de clasificación de Venieratos y Anagnostopoulou, 11 de 14 casos (79%) mostró comunicaciones de tipo I, dos de 14 (14%) mostraron el tipo II, y el restante (7%) mostró un patrón de comunicación de tipo III. CONCLUSIÓN: El conocimiento previo de las comunicaciones entre estos dos nervios vecinos, en términos tanto de sus incidencias como patrones de comunicación, puede ser de una importancia considerable para los neurólogos y ortopédicos a la hora de tratar con el síndrome del atrapamiento del nervio en la extremidad superior de los pacientes.

ABSTRACT

La distribución de los ramos nerviosos sensitivos en el borde lateral y en el dorso de la mano han sido descritos con mayor exactitud en las últimas décadas, debido al avance de nuevas técnicas de diagnóstico, las cuales han permitido detectar que alrededor del 40 por ciento de la población examinada presenta algún grado de variación anatómica en el territorio de distribución de los nervios involucrados. Conocer el número de fibras que componen un ramo nervioso cutáneo de la región de la mano, ha adquirido mayor relevancia con el desarrollo de técnicas de microcirugía y de ultrasonografía, procedimientos que han demostrado la utilidad de este conocimiento en el diagnóstico y tratamiento de las lesiones nerviosas. Así, la arquitectura fascicular, el área adiposa y el área vascular de un ramo nervioso determinado constituyen datos que se ha demostrado se modifican con la edad y, en consecuencia, van condicionar la conducta terapéutica y el pronóstico de las lesiones nerviosas. En este caso presentamos una variación anatómica bilateral extremadamente rara, que involucra al ramo superficial del nervio radial y al nervio cutáneo lateral antebraquial; situación que aparece descrita en la literatura especializada sólo una vez y que modifica notablemente la inervación sensitiva del borde radial de la mano...


The distribution of sensory nerve branches in the lateral and the back of the hand have been described more accurately in recent decades due to advances in new diagnostic techniques, which have identified that about 40 percent of the population examined have some degree of anatomical variation in the distribution area of the sensitive nerves involved. The knowledge of the number of fibers forming a sensitive nerve of the hand has become more important with the development of microsurgical techniques and ultrasonography; procedures that have demonstrated the usefulness of this information in the diagnosis and treatment of nerve injuries. Thus, the fascicular architecture, adipose tissue area and the vascular area of a nerve branch, data that has been demonstrated that change with age, will determine the therapeutic and prognosis of nerve injuries. In this case we present an extremely rare and bilateral anatomical variation, involving the superficial branch of radial nerve and the lateral antebrachial cutaneous nerve, a situation that is described in the literature only once and which notably alter the sensory innervations of the radial edge of the hand...

ABSTRACT

El presente trabajo se propone comparar los diámetros de los vientres del músculo bíceps braquial y músculo braquial con el objeto de esclarecer algunas dudas sobre cuál sería el músculo más potente en la flexión del antebrazo. Las comparaciones fueron realizadas en 50 miembros superiores de cadáveres fijados. Se hizo evidente la diferencia promedio de 0,51 cm (5,8 por ciento) en el diámetro del vientre muscular del músculo braquial, ratificando lo encontrado en la revisión bibliográfica, donde el músculo braquial es un importante flexor del codo.


The present work seeks to compare the diameter of the wombs of the muscle brachial biceps and m. brachial in the intention of settling the doubts on which would be the most potent muscle in the flexing of the forearm. The measures were accomplished in 50 superior members of fastened corpses. A medium difference of 0.51 cm (5.8 percent) was evident in the diameter of the muscular womb the largest for m. brachial, ratifying what was found in the bibliographical revision, that brachial muscle is an important flexor of the elbow.

ABSTRACT

La distribución de los ramos nerviosos sensitivos en el borde lateral y en el dorso de la mano han sido descritos con mayor exactitud en las últimas décadas, debido al avance de las técnicas de electrodiagnóstico que ofrecen un alto rendimiento, las cuales han permitido detectar que alrededor del 40 por ciento de la población examinada presenta algún grado de variación anatómica en el territorio de distribución de los nervios involucrados. En este caso presentamos una variación anatómica bilateral extremadamente rara, que involucra al ramo superficial del nervio radial (NRS) y al nervio cutáneo antebraquial lateral (CABL); donde NRS se conecta de forma íntegra con el ramo medial de CABL, formándose así un tronco común (TC) que se distribuye por la región dorsal de la mano. Por su parte, el ramo lateral de CABL se distribuye por el borde lateral de la mano, ocupando el territorio cutáneo de NRS; situación que aparece descrita en la literatura especializada sólo una vez. El hallazgo de estas variaciones anatómicas en los cadáveres disecados con fines docentes en nuestro Departamento de Anatomía, tienen un valor formativo indiscutible para nuestros alumnos de pregrado y especialmente para los de postgrado, quienes pueden comprobar de primera mano la enorme variabilidad del ser humano, valorando las implicancias en la clínica diaria de este conocimiento anatómico.


The distribution of the sensory nerve branches in the lateral and the back of the hand have been described more accurately in the past decades due to advancement of high performance electro-diagnostic variation techniques, which indicate that approximately 40 percent of the population examined have some degree of anatomical variation in the distribution area of the nerves involved. In this case we present an extremely rare, bilaterally detected variation, involving the superficial branch of the radial nerve (SBRN) and lateral antebrachial cutaneous nerve of the forearm (LABCN), where the SBRN is connected integrally with the medial branch of LABCN, forming a common trunk (CT) distributed by the dorsal region of the hand. Furthermore, the lateral branch of the LABCN is distributed in the lateral border of the hand, occupying the area of the skin of the SBRN, an event that is described only once in the literature. The discovery of these anatomical variations in dissected cadavers for teaching purposes, in the Department of Anatomy, have an undeniable educational value for our undergraduate students and especially for the graduate who can observe the enormous variability of human beings first hand, and value implications of this anatomical knowledge in daily clinic.

ABSTRACT

El uso racional del acceso a las venas superficiales en la fosa cubital es de especial relevancia en pacientes pediátricos, con fístulas para diálisis, en quimioterapia, o como vías para acceso venoso central. Se han descrito varios patrones venosos con sus beneficios o riesgos. Se determinó la frecuencia de los patrones venosos en una población de ambos sexos nacida en Bucaramanga, Colombia, en 800 miembros superiores (200 hombres y 200 mujeres), conforme a la clasificación propuesta por del Sol et al. (1988, 2007). El análisis descriptivo para variables cualitativas nominales y ordinales mediante porcentajes, las cuantitativas con media y desviación estándar en el caso de variables con distribución normal y las no paramétricas identificadas con número mediante mediana y rango intercuartil. Se comparó género y patrón venoso mediante la prueba de Kwallis y chi cuadrado. El patrón predominante fue el III, 297 casos (37 por ciento) determinado por una alta frecuencia en mujeres (54 por ciento), seguido del II 190 casos (24 por ciento). En el lado derecho para ambos géneros el patrón más frecuente fue el III, 141 casos (35 por ciento), seguidos del II, 89 casos (22 por ciento). En el lado izquierdo para ambos géneros predominó el patrón III, 156 casos (39 por ciento), seguido por el II con 101 casos (25 por ciento). El patrón II fue el mayor en hombres (28 por ciento), seguido del III (21 por ciento). En mujeres predominó el patrón III, seguido del II (19 por ciento). En el lado derecho en hombres, predominó el II (24 por ciento) seguido del IV (23 por ciento). En el lado derecho en mujeres predominó el III (51 por ciento) seguido del II (20 por ciento). En el lado izquierdo en hombres, predominó el patrón II (32 por ciento) seguido del III (21 por ciento). En el lado izquierdo en mujeres predominó III (57 por ciento) seguido del II (18 por ciento). El patrón que en más ocasiones se presentó al mismo tiempo en ambos lados en la misma persona...


The rational use of access to the superficial veins in the cubital fossa is particularly important in pediatric patients, with fistulas for dialysis, chemotherapy, or central venous access routes. Several venous patterns have been described for their benefits or risks. The prevalence of venous patterns in a population of both sexes born in Bucaramanga, Colombia, in 800 upper limbs (200 men and 200 women), according to the classification proposed by del Sol et al. (1988, 2007). The descriptive analysis for nominal and ordinal qualitative variables with percentages, the quantitative mean and standard deviation for normally distributed variables and nonparametric identified by number using median and interquartile range, was compared by gender and venous pattern through test and chi square Kwallis. The predominant pattern was III, 297 cases (37 percent) determined by a high frequency in women (54 percent), followed by II 190 cases (24 percent). On the right side for both sexes the most common pattern was III, 141 cases (35 percent), followed by II, 89 cases (22 percent). On the left side for both genders predominant pattern III, 156 cases (39 percent), followed by the second with 101 cases (25 percent). Pattern II was greater in men (28 percent), followed by III (21 percent). In women the pattern III predominated, followed by II (19 percent). On the right side in men, II predominated (24 percent) followed by IV (23 percent). On the right side III predominated in women (51 percent) followed by II (20 percent). On the left side in men, the predominant pattern II (32 percent) followed by III (21 percent). On the left side in women predominated III (57 percent) followed by II (18 percent). The pattern appeared more often at the same time on both sides in the same person was III (16.6 percent).

ABSTRACT

Neuro-vascular entrapments associated with variations observed in the origins of muscles in the arm are not uncommon. Though additional heads of biceps brachii muscle and extra fibres of brachialis muscles have been demonstrated earlier, bilateral additional heads of the biceps are rarely seen, especially with entrapment of the median nerve and the brachial arteries in both the arms. The present study reports conspicuous heads of the biceps brachii originating extensively from the medial inter-muscular septum, sharing its origin with the brachialis muscle. The extra origins of the muscle formed long musculo-aponeurotic tunnels. The tunnels measured eight centimeters in length extending from the lower arm to the cubital-fossa. Both the median nerve and the brachial arteries passed through the tunnel. The lower aspect of the left tunnel exhibited origins of fibres belonging to the superficial flexors of the forearm. The neuro-vascular structures did not give any branches in the tunnel. Awareness of such variations can aid clinicians in diagnosing and treating such neuropathies and vascular compromise.


No es infrecuente observar atrapamientos neuro-vasculares asociados con variaciones en el origen de los músculos del brazo. A pesar de haberse observado cabezas adicionales del músculo bíceps braquial y fibras extra del músculo braquial raramente estas cabezas adicionales bilaterales han sido causantes de la compresión del nervio mediano y de la arteria braquial. En este trabajo presentamos las cabezas del músculo bíceps braquial originándose en gran parte en el tabique intermuscular medial compartiendo su origen con el músculo braquial. Los orígenes adicionales del músculo forman largos túneles músculo fasciales. Los túneles miden 8 cm de longitud, y se extienden desde la parte inferior del brazo hasta la fosa cubital. Tanto el nervio mediano como la arteria braquial pasan por el túnel. La parte inferior del túnel izquierdo dio origen a fibras pertenecientes al músculo flexor superficial del antebrazo. Las estructuras neurovasculares no otorgaron colaterales en el túnel. El conocimiento de estas variaciones puede ayudar a los clínicos en el diagnóstico y el tratamiento de neuropatías y compromiso vascular.

ABSTRACT

Introducción: La resección transaxilar con apoyo endoscópico es una técnica quirúrgica novedosa en el manejo del síndrome de salida de tórax que permite identificar con seguridad el paquete vasculonervioso y las diferentes estructuras. Nuestro objetivo fue valorar los resultados de morbilidad y mortalidad del tratamiento quirúrgico en el síndrome de salida de tórax con esta técnica. Material y métodos: Estudio longitudinal prospectivo de 22 intervenciones quirúrgicas de resección transaxilar de la primera costilla, con apoyo endoscópico a pacientes con síndrome de salida de tórax, entre enero de 2000 y enero de 2009, en un hospital privado de la cuidad de Puebla, México. De ellos, 16 mujeres y seis hombres, con edad promedio de 35 años. Encontramos 16/22 (72.7 %) pacientes con síntomas neurológicos y 6/22 (27.3 %) con síntomas venosos; entre ellos, 2/22 (9.09 %) presentaban trombosis venosa de esfuerzo axilosubclavia. Resultados: Entre las 22 intervenciones encontramos bandas fibrosas en 8/22 pacientes (36.3 %): 1/22 (4.5 %) con una banda cervical tipo 1 y costilla cervical grado II, 1/22 (4.5 %) con banda cervical tipo 2 y costilla cervical grado I, 2/22 (9.09 %) con banda tipo 3, 1/22 (4.5 %) con banda tipo IV, 1/22 (4.5 %) con banda tipo V y 2/22 (9.09 %) con banda tipo VII y trombosis axilosubclavia. Después del procedimiento, 20/22 (90.9 %) pacientes manifestaron mejoría total y 2/22 (9.1 %), parestesias leves. No hubo complicaciones por lesiones nerviosas, vasculares o pleurales. Conclusiones: La técnica escrita provee un margen de seguridad amplio y mejora la visibilidad, disminuyendo las complicaciones quirúrgicas.


BACKGROUND: Endoscopic-assisted transaxillary first rib resection is a novel approach in the management of thoracic outlet syndrome (TOS) and allows us to safely identify the neurovascular package and different structures. Our main objective is to assess the results of morbidity and mortality of the surgical treatment in TOS with this technique. METHODS: We carried out a prospective, longitudinal study with 22 surgical interventions of transaxillary first rib resection with endoscopic support in patients with TOS from January 2000 to January 2009 in a private hospital located in Puebla, Mexico. There were 16 females and six males with a mean age of 35 years. We found 16/22 (72.7%) patients with neurological symptoms and 6/22 (27.3%) with venous symptoms; 2/22 (9.09%) patients had effort thrombosis of the axillary-subclavian vein. RESULTS: Of the 22 interventions, we found fibrous bands in 8/22 patients (36.3%); 1/22 (4.5%) with type 1 cervical band and cervical rib grade II, 1/22 (4.5%) with type 2 cervical band and cervical rib grade I, 2/22 (9.09%) with band type 3, 1/22 (4.5%) with band type IV, 1/22 (4.5%) with band type V and 2/22 (9.09%) with band type VII and axillary-subclavian thrombosis. After the procedure, 20/22 (90.9%) patients showed total symptom improvement and 2/22 (9.1%) patients had mild paresthesias. There were no complications from nerve, vascular or pleural damage. CONCLUSIONS: This technique provides an ample margin of safety and improves visibility, reducing surgical complications.

ABSTRACT

OBJETIVO: identificar a alteração de sensibilidade no trajeto do nervo intercostobraquial utilizando um estesiômetro e observar a repetição das medições efetuadas com esse aparelho. MÉTODOS: foi aplicado o estesiômetro de Semmes-Weinstein para a avaliação da sensibilidade no trajeto do nervo intercostobraquial. Participaram desse estudo 94 mulheres divididas em dois grupos: Grupo CA, composto por 47 mulheres submetidas à linfonodenectomia axilar por câncer de mama, e grupo comparativo, composto por 47 mulheres sem câncer de mama e que não foram submetidas a qualquer tipo de cirurgia nas axilas. Em cada participante, foram realizadas anamnese e aplicação do estesiômetro duas vezes consecutivas. As respostas ao teste com estesiômetro do Grupo Controle foram utilizadas como valores de referência de normalidade. RESULTADOS: a prevalência de alteração de sensibilidade, no Grupo CA, foi de 85,1 por cento, com base nas repostas do Grupo Controle. Foi confirmada a repetição na aplicação do estesiômetro no Grupo CA por meio do teste de Kappa (p=0,8). CONCLUSÕES: na amostra desse estudo, as alterações de sensibilidade tiveram alta prevalência; as avaliações efetuadas com o uso do estesiômetro apresentaram repetição e, por isto, considerou-se o equipamento confiável para avaliação da sensibilidade no trajeto do nervo intercostobraquial.


PURPOSE: to identify sensitivity alteration in the intercostal brachial nerve pathway using an extensiometer, and to observe the measurement reproducibility of the apparatus. METHODS: the Semmes-Weinstein extensiometer was used to evaluate the sensitivity along the intercostal brachial nerve pathway. Ninety-four women have participated in the study, divided into two groups: a CA Group composed of 47 women submitted to breast cancer axillary lymphadenectomy, and a comparative group composed of 47 women without breast cancer, who had not been submitted to any kind of axillary surgery. Each participant underwent anamnesis and two consecutive applications of the extensiometer. The Control Group responses to the extensiometer test were used as normality reference values. RESULTS: based on Control Group responses, the prevalence of sensitivity changes was 85.1 percent in the CA Group. Reproducibility of the extensiometer application was confirmed in the CA Group through the Kappa's test (p=0.8). CONCLUSIONS: in this studied sample, sensitivity alterations had high prevalence; evaluations made with the extensiometer were reproducible, and thus we consider the equipment reliable to evaluate sensitivity along the intercostal brachial nerve pathway.

ABSTRACT

There is substantial controversy in literature about human dermatomes. In this work, C5 and C6 superior limb dermatomes were studied. The method consisted of comparing clinical signs and symptoms with conduction studies, electromyographical data, neurosurgical findings, and imaging findings obtained by computerized tomography (CT) or magnetic resonance imaging (MRI), for each patient. Data analysis from superior members in 18 patients suggests that C5 is located in the lateral aspect of the shoulder and arm, and C6 in the lateral aspect of the forearm and 1st, 2nd, and 3rd fingers. To our knowledge this is the first time that C5 and C6 human dermatomes have been studied by all the following methods together: clinical, electromyographical, CT and MR imaging, and surgical findings.


Há controvérsias na literatura sobre os dermátomos humanos. Neste estudo os dermátomos do membro superior C5 e C6 foram analisados. O método consistiu em comparar os sinais e sintomas com achados eletromiográficos, de imagem e achados cirúrgicos. Análise dos dados do membro superior de 18 pacientes sugere que o dermátomo C5 esteja localizado na região lateral do ombro e braço, e o dermátomo C6 na região lateral do antebraço e 1º, 2º e 3º dedos da mão. Este é o primeiro estudo em que os dermátomos C5 e C6 foram avaliados pelos dados clínicos, eletromiográficos, de imagem e achados cirúrgicos.

ABSTRACT

OBJECTIVE: To report on 9 patients presenting with sporadic motor neuron disease , who over a long period of time evolved with a symmetrical proximal brachial amyotrophic diplegia. METHODS: Nine patients were followed-up who , displayed, since onset, a progressive limitation of arm flexion/abduction resulting in a peculiar posture with both hands hanging loosely beside the trunk. Electrophysiological test results were consistent with lower motor neuron disease. Cervical MRI was performed in all patients. RESULTS: Nine male subjects with ages ranging from 38 to 73 years at onset of symptoms, developed bilateral and symmetric paresis and atrophy of upper limb muscles. Proximal muscles were more involved than the distal groups. In most patients tendon reflexes were absent or hypoactive in the upper limbs. Needle electromyography (EMG) revealed positive sharp waves and fibrillations and high amplitude polyphasic potentials with an incomplete recruitment pattern in most upper limb muscles. EMG of lower limb muscles was normal in some cases while abnormal in others. MRC did not disclose cervical spinal cord abnormalities from C5-T1. CONCLUSION: Attention is called to the Man-in-the-Barrel syndrome in some motor neuron diseases, especially in patients with progressive spinal atrophy and amyotrophic lateral sclerosis.


OBJETIVO: Relatar nove pacientes com a doença do neurônio de motor esporádica que apresentam progressiva e simétrica diplegia braquial amiotrófica. MÉTODOS: Acompanhamos nove pacientes que exibiram, desde o começo, uma limitação progressiva de flexão/abdução do membro superior resultando em uma postura peculiar com ambas as mãos pendentes ao longo do tronco. Os resultados dos testes eletrofisiológicos foram consistentes com achados de doença do neurônio motor inferior. Ressonância magnética (RM) cervical foi realizada em todos pacientes. RESULTADOS: Nove pacientes do sexo masculino com idades entre 38-73 anos no início dos sintomas desenvolveram paresia e a atrofia bilateral simétrica dos músculos dos membros superiores. Os músculos proximais foram mais comprometidos do que os distais. Os reflexos tendíneos dos membros superiores encontravam-se abolidos ou hipoativos na maioria dos pacientes. A eletromiografia (EMG) revelou ondas positivas, fibrilações e potenciais polifásicos de alta amplitude com padrão incompleto de recrutamento na maioria dos músculos dos membros superiores. A EMG dos membros inferiores foi normal em alguns casos e anormal em outros. A RM não evidenciou anormalidades da medula espinhal cervical de C5-T1. CONCLUSÃO: Atentamos para a síndrome do homem do Barril em algumas doenças do neurônio de motor, especialmente na atrofia espinhal progressiva e esclerose lateral amiotrófica.

ABSTRACT

O propósito deste estudo foi avaliar, através de uma série de casos, a relação entre tensão neural adversa do nervo mediano (TNAm) e o parâmetro eletrofisiológico em 38 pacientes com sintomas da síndrome do túnel do carpo (STC), submetidos a estudos de condução nervosa (ECN). As principais medidas foram a TNAm obtida no teste de provocação de tensão neural (TPTN) e parâmetros dos ECN, dividindo-se os braços avaliados em três grupos: normal, com alteração eletrofisiológica sem gravidade e com alteração eletrofisiológica grave. Correlação significante entre TNAm e parâmetros dos ECN foram encontrados (p<0,05), bem como entre a TNAm e os três grupos definidos pela alteração eletrofisiológica (r s=+0.437, p=0,002). Valores de TNAm foram significantemente maiores nos braços com diagnóstico eletrofisiológico (p=0,007). Sugere-se que a TNAm tem participação na fisiopatologia da STC, indicando o uso de procedimentos terapêuticos que diminuam ou previnam a tensão neural.
